STEM Minds Online Academy Pilot Consent
Your participation should be entirely voluntary; if you decide not to participate in the pilot, you will not be penalized
The information collected below will be kept confidential. Except as required by law, no information that could directly or indirectly identify you will be released or published without your prior explicit consent.
When participating in the STEM Minds Online Academy Pilot, participants have the option to complete as many or as few sections of the program as they would like. You will not be penalized in any way for failing to complete the program.
In order to participate in the pilot, the following information will be requested:
- Parent/Guardian first and last name
- Child first and last name
The parent/guardian first and last name is requested in order to track consent to participate in this pilot. The child’s first and last name will be used to create their unique login ID. Your email is requested in order to contact you about the STEM Minds Online Academy as per your participation in this pilot.
As part of your participation in the STEM Minds Online Academy Pilot, you and your child will be requested to provide feedback through an online survey related to student engagement with and enjoyment of the platform. You will not be penalized in any way for failing to complete the survey.
Based on our current knowledge, there are no new risks involved in this project other than those already associated with normal computer use. If side effects or consequences are discovered during the pilot, you will be informed immediately. Participants will be free to withdraw from the pilot at any time with no penalties.
It is possible that STEM Minds Corp. may develop a commercial product as a result of this research. If this is the case, you will receive no direct or indirect benefits from this product. By signing the consent form, you are not waiving any of your legal rights.
